Why is iron deficiency so common in our region?

Iron deficiency anaemia is the most widespread nutritional disorder in the world, and the Arab world and Middle East rank among the regions with the highest prevalence. A systematic review published in the Journal of Clinical Medicine in 2026 found that the rates of anaemia and iron deficiency in Saudi Arabia are strikingly high, particularly among women of reproductive age and pregnant women.

Several factors explain this in our region. Many households rely primarily on grains and pulses as their main protein source rather than meat, which means most dietary iron comes from plant foods with low absorption rates. The widespread custom of drinking tea with or immediately after meals in countries such as Egypt, Syria, Sudan and Iraq is a significant and often unrecognised cause of iron loss. Women's iron requirements are also substantially higher during the years of menstruation, pregnancy and breastfeeding.

An important distinction: iron deficiency does not always produce obvious anaemia on a blood count. Your ferritin (Ferritin), which measures iron stores, can be low and cause fatigue, poor concentration and hair loss even while your haemoglobin (Haemoglobin) remains within the normal range. A ferritin test is therefore a more sensitive measure of your iron status than haemoglobin alone, and worth asking for if you have relevant symptoms.

Haem and non-haem iron: why does the difference matter so much?

Dietary iron comes in two fundamentally different forms with very different absorption rates. Haem iron (Haem iron) is found in red meat, poultry, fish and shellfish, where it is bound to the proteins haemoglobin and myoglobin in animal tissue. This form is absorbed in the gut with an efficiency of 15 to 35 percent, and it is relatively unaffected by other foods eaten in the same meal.

Non-haem iron (Non-haem iron) is found in plant sources such as lentils, beans, spinach, nuts and fortified cereals, as well as in eggs. Its absorption rate is only 2 to 20 percent, and it is strongly affected by what else you eat in the same meal. This is why knowing what enhances and what blocks absorption is not optional information if you rely heavily on plant foods.

A comprehensive review published in ACS Omega in 2022 gathered extensive evidence on iron absorption factors and found that the gap between the best and worst dietary scenario for non-haem iron absorption can be up to tenfold. The same quantity of plant iron in two different meals might be 2 percent absorbed in one and 20 percent in another, depending entirely on the composition of the meal.

The best iron sources in Arab cuisine

Liver, particularly calf or chicken liver, is among the richest haem iron sources in Arab cooking, and it appears across regional cuisines from Egyptian liver with peppers and vinegar to Levantine liver cooked with garlic and lemon. 100 grams of cooked calf liver provides approximately 6 to 7 milligrams of iron, representing roughly a third to a half of the daily requirement for a woman of reproductive age.

Red meat from lamb and beef, the basis of dishes like mansaf, kabsa and mahshi across the Arab world, provides high-quality haem iron. Seafood, particularly oysters, octopus and shrimp, is another excellent source, and forms an important part of the cooking traditions of the Gulf coasts, Yemen and Morocco. Oysters in particular are among the richest single iron sources of any food.

From plant sources in Arab cooking: lentils are a cornerstone in Syrian, Iraqi, Egyptian and Lebanese food. Beans in their many forms are central to Sudanese and Egyptian cuisine. Chickpeas (hummus) are eaten across the region. These are genuinely good iron sources, but getting the iron into your bloodstream requires the right dietary strategy alongside them.

Tea and iron: the most important dietary habit to change

The habit of drinking tea with meals is one of the most significant dietary factors reducing iron absorption across the Arab world. A study in Critical Reviews in Food Science and Nutrition in 2000 documented the mechanism clearly: tea contains compounds called tannins and other polyphenols that form chemical bonds with non-haem iron in the gut and prevent it from being absorbed.

A key study published in the British Journal of Nutrition found that tea drunk with a meal rich in non-haem iron reduced iron absorption from that meal by between 60 and 90 percent. This is a very large effect. If your main meal is built around lentils or beans and you drink tea with it or shortly after, you are absorbing only a fraction of the iron that meal contains.

The solution is not to stop drinking tea but to change the timing. Wait one to two hours after a meal before drinking tea, or drink it at least one hour before you eat. This simple adjustment to a daily habit can make a measurable difference to iron stores over time, especially for those who are already deficient.

Black tea has a stronger effect than green tea due to a higher tannin concentration, but both have a meaningful inhibitory effect on non-haem iron absorption. Coffee also contains polyphenols with a similar, though somewhat weaker, effect on iron. The same timing advice applies to coffee for people managing low iron.

Vitamin C: the most effective tool for improving plant iron absorption

Vitamin C (Ascorbic acid) converts non-haem iron from its ferric form, which is poorly absorbed, to its ferrous form, which the gut absorbs readily. Including a source of vitamin C in a plant-based iron-rich meal substantially raises the amount of iron that reaches your bloodstream.

In Arab cooking, this is easily done: lemon juice squeezed over a lentil dish or ful medames, tomatoes cooked into a bean stew, fresh parsley and coriander in side salads, a glass of orange or grapefruit juice alongside a breakfast of fortified grain porridge. Many of these are traditional combinations in Arab cooking that were nutritionally wise long before anyone understood the biochemistry.

Eating meat alongside pulses in the same meal has a doubly positive effect: the haem iron from the meat is absorbed efficiently, and the animal protein also enhances absorption of the non-haem iron from the accompanying pulses. A review in the American Journal of Clinical Nutrition in 2010 confirmed this combined effect of animal protein on plant iron absorption, making dishes like lentils with a small amount of meat a genuinely good iron strategy.

Other things that reduce iron absorption

Tea is not the only inhibitor. Phytates, found in wheat, rice, beans and lentils themselves, also reduce non-haem iron absorption significantly. Soaking lentils and beans for several hours before cooking, and boiling them thoroughly, reduces their phytate content and improves the iron you can absorb from them. This is another traditional practice in Arab cooking that carries real nutritional logic.

Calcium in large amounts also inhibits non-haem iron absorption. Drinking a large glass of full-fat milk with a plant-based iron-rich meal has a measurable blocking effect. This does not mean avoiding dairy, but it does mean being mindful about not drinking large amounts of milk alongside a meal you are specifically relying on for iron.

Coffee, like tea, contains polyphenols that reduce non-haem iron absorption, though generally to a somewhat lesser degree than strong black tea. For people actively managing low iron, the same timing principle applies: coffee before meals or two hours after is better than coffee with food.

Daily iron requirements: who needs more?

Daily iron requirements vary substantially with age, sex and health status. A woman of reproductive age needs around 18 milligrams per day because of the iron lost in menstrual blood. A pregnant woman needs approximately 27 milligrams per day, because she is also building the baby's blood supply. An adult man and a post-menopausal woman need only around 8 milligrams per day.

A 2024 cross-sectional study in BMC Public Health of pregnant women in Jordan found that dietary diversity was strongly associated with lower risk of iron deficiency anaemia. A pregnant woman eating a varied diet that includes animal sources, multiple types of pulses and fresh fruit was in significantly better iron status than one relying on one or two food types. The diversity matters as much as the total quantity.

Children also need close attention during rapid growth phases, particularly between 6 months and 2 years and again during adolescence. Breast milk alone does not cover iron needs after 6 months, and iron-rich complementary foods, including mashed liver, egg yolk and well-cooked mashed pulses, should be introduced at that point.

When to speak to a doctor about iron deficiency

Improving your diet is important and effective, but it is not sufficient on its own when iron deficiency is already advanced. If you have symptoms including persistent unexplained tiredness, shortness of breath on mild exertion, visible paleness in the face or inside the lower eyelid, frequent mouth ulcers, noticeable hair loss, or brittle nails, these point to a blood test rather than just dietary adjustment.

Your doctor will likely order ferritin, haemoglobin and possibly a full blood count (CBC). If iron deficiency is confirmed, an iron supplement is usually needed because diet alone is too slow to replenish significantly depleted stores. That does not make diet irrelevant: a good iron-focused diet is essential for maintaining levels once a supplement has restored them, and for preventing recurrence.

An important caution: do not take iron supplements without medical guidance. Iron is toxic in excess and can accumulate in organs when taken unnecessarily. Start iron supplements only when a blood test has confirmed real deficiency and a doctor has recommended them, and take them in the dosage and for the duration your doctor specifies.

Frequently asked questions

Is spinach actually a good iron source, as is often claimed?

Spinach does contain a reasonable amount of iron by weight, but it also contains compounds called oxalates that impair its absorption, making the effective iron yield lower than the raw figure suggests. Adding lemon juice helps somewhat, but spinach is not among the most efficient iron sources in practice. Lentils, beans and liver are more reliable choices.

Can I meet my iron needs from an entirely plant-based diet?

It is possible but requires careful planning. A vegan diet relies entirely on non-haem iron with its lower absorption rate, so larger quantities of iron-rich plant foods are needed, vitamin C should be included at every iron-rich meal, and inhibitors like tea should be carefully timed. Regular ferritin monitoring with a doctor is strongly advisable.

Does cooking in cast-iron pans add iron to food?

Yes, cooking acidic foods such as tomato sauce in cast-iron cookware does raise the iron content of the food meaningfully. However, this iron is mostly non-haem and subject to the same absorption factors. It is a worthwhile additional contribution, not a complete solution to iron deficiency.

Is dates a good iron source as is traditionally believed in the region?

Dates contain a moderate amount of non-haem iron and are an excellent food for their fibre and mineral content, but the widespread belief that they are a treatment for anaemia is an overstatement. Dates are a valuable part of a balanced diet, but they are not a substitute for liver, meat or pulses when iron is what is needed.

How long should I wait between a meal and drinking tea?

The most protective gap is one to two hours after a meal before drinking tea. Tea drunk at least one hour before eating affects meal iron absorption less significantly. The practical rule: never drink tea with or immediately after a meal built around plant-based iron sources.

Is tea with milk less harmful to iron absorption than black tea?

No. Adding milk to tea does not meaningfully cancel the tannins' inhibitory effect on iron absorption. Some research suggests milk binds to tannins to a partial degree, but the blocking effect on non-haem iron remains significant regardless of whether the tea is taken with or without milk.

Do I need an iron supplement if I improve my diet?
