What is ferritin and why does it matter more than a direct iron test?
Ferritin is a protein found mainly inside the cells of the liver, spleen and bone marrow. Its job is to store iron safely and release it when the body needs it. A small amount of ferritin leaks into the bloodstream at all times, and the level of that circulating ferritin is a reliable window onto your total iron reserves across all the body's tissues, not just what is floating in the blood right now.
A standard iron blood test measures only the iron circulating freely in your blood at that moment. That number is heavily influenced by what you ate earlier in the day and can swing widely from one sample to the next, making it an unreliable guide to your actual stores. Ferritin, by contrast, reflects weeks and months of iron storage in the tissues, making it a much more stable and meaningful measurement.
This is why doctors now use ferritin to diagnose iron depletion before it ever shows up as frank anaemia. A person can have empty stores, feel exhausted and notice their hair falling out, while their red blood cells and haemoglobin still look entirely normal on a standard blood count. At that early stage, ferritin is the only test that reveals what is actually happening.

How to read your ferritin result
The normal range quoted by most laboratories is roughly 12 to 150 ng/mL for women and 12 to 300 ng/mL for men, though different laboratories may use slightly different cut-offs. Some labs report in mcg/L, which is the same number. Always check the reference range printed on your own result sheet alongside the value, rather than comparing your number to a range you read somewhere else.
A ferritin below 12 ng/mL signals clearly depleted iron stores. A result between 12 and 30 ng/mL sits in a borderline zone that many doctors treat as insufficient, particularly in women who are pregnant, breastfeeding, or experiencing symptoms such as persistent fatigue or noticeable hair loss. In these circumstances, the symptoms and the clinical picture matter as much as where exactly the number falls.
At the upper end, a ferritin above 300 ng/mL can indicate chronic inflammation, liver disease, or, rarely, hereditary haemochromatosis, a condition in which the body absorbs and accumulates too much iron over years. A markedly raised ferritin should not be ignored, and your doctor will want to understand what is driving it rather than simply reassuring you that your iron is adequate.
The big trap: inflammation raises ferritin and hides iron deficiency
This is the point that misleads many people, and it is the main reason a ferritin result cannot always be read in isolation. Ferritin is not only raised when iron stores are full. It is also raised whenever there is active inflammation anywhere in the body: a bacterial or viral infection, a chronic autoimmune condition such as rheumatoid arthritis, liver disease, or any other ongoing inflammatory process.
In practice, a person with genuine iron deficiency can obtain a normal or even raised ferritin result if inflammation is present at the same time, because the inflamed tissue is secreting ferritin regardless of the actual iron stores. A 2017 paper in the American Journal of Clinical Nutrition, part of the international BRINDA project, demonstrated that this effect is large enough to mask iron deficiency across entire population groups when ferritin is used without an inflammation marker alongside it.
The practical solution is straightforward: ask your doctor to run a CRP (C-reactive protein) test alongside the ferritin. If CRP is elevated, the ferritin result becomes difficult to interpret on its own, and additional tests such as transferrin saturation and haemoglobin are needed to build a complete and accurate picture. Do not accept a normal ferritin as definitive proof of adequate iron if you still have symptoms.
Why iron deficiency is so common in this region
A 2021 review published in the International Journal of Environmental Research and Public Health, covering countries of the Eastern Mediterranean, found that anaemia remains a significant and persistent public health problem, particularly among women of reproductive age and children under five. A 2025 analysis tracking two decades of trends in the same region confirmed the burden has not declined as hoped despite multiple national programmes.
The local drivers are several and often act together. Heavy or prolonged menstrual periods, closely spaced pregnancies and extended breastfeeding all draw heavily on iron reserves. Diets rich in tea and coffee consumed with or immediately after meals contain tannins and polyphenols that substantially reduce iron absorption from plant sources. Where diets rely mainly on bread, legumes and vegetables, the iron present is mostly non-haem iron, which the body absorbs far less efficiently than the haem iron in red meat.
In countries affected by conflict or disrupted health systems, intestinal parasites also contribute by causing chronic low-grade blood loss or competing for nutrients in the gut. In these settings, iron deficiency may have several simultaneous causes, and supplementation alone will not resolve the problem without addressing the underlying driver. A careful history and sometimes stool examination are part of the workup.
Signs of low iron stores before anaemia appears
Iron depletion moves through stages. In the first stage, stores fall but haemoglobin stays normal. At this point you might notice unexplained tiredness that is different from ordinary fatigue, poor concentration and working memory, increased hair shedding beyond what you consider normal, brittle or ridged nails, or occasionally an odd craving to chew ice, chalk or non-food substances. This last symptom, called pica, is a well-recognised marker of iron deficiency.
In the second stage, haemoglobin begins to fall and frank anaemia develops, with pallor, pronounced fatigue, palpitations and breathlessness on mild exertion such as climbing stairs or walking briskly. Many women reach this stage without ever connecting their symptoms to a nutritional deficiency, attributing them instead to the demands of daily life, early waking with young children, or ageing.
Ferritin catches the problem at stage one, before any drop in haemoglobin is visible on a standard blood count. Treating iron depletion at this early stage is faster and easier than correcting established anaemia, and it avoids the months of impaired concentration, reduced work capacity and heavy fatigue that accompany the more advanced stage.

What other tests does your doctor need alongside ferritin?
Ferritin alone is not always sufficient for a complete picture, particularly when inflammation may be distorting the result. If your ferritin is low, your doctor will usually request a full blood count (CBC), a serum iron level, total iron binding capacity (TIBC) and transferrin saturation to confirm the diagnosis and gauge how advanced the deficiency is. These four tests together give a consistent and reliable signal that ferritin alone cannot provide when CRP is elevated.
If ferritin is unexpectedly high above 300 ng/mL with no obvious recent illness or known inflammatory condition, your doctor may want liver function tests, serum iron and transferrin measurements, and occasionally a genetic test to exclude hereditary haemochromatosis. This inherited condition causes gradual iron overload that damages the liver, heart and joints over decades, but it responds well to regular therapeutic venesection if caught before organ damage has occurred.
Pregnancy deserves a special mention. Ferritin naturally falls during pregnancy because blood volume expands rapidly, diluting the concentration in the blood even when iron stores are not truly depleted. Most obstetricians check ferritin in the first trimester and again in the third, because iron deficiency in late pregnancy is linked to preterm birth, low birth weight and impaired fetal brain development in the critical last weeks before delivery.

How iron deficiency is treated
For mild to moderate deficiency, oral iron supplements are effective in most cases when taken correctly. Take them on an empty stomach if you can tolerate it, or with a glass of orange juice, since vitamin C significantly enhances the absorption of non-haem iron. Avoid taking iron at the same time as tea, coffee, milk, calcium supplements or antacids, all of which substantially reduce absorption by chelating the iron in the gut before it reaches the intestinal wall.
Continue iron supplements for three to six months after your haemoglobin returns to normal. This is longer than most people expect, because replenishing ferritin stores in the tissues takes considerably longer than raising haemoglobin in the blood. Stopping treatment as soon as you feel better, before stores are fully rebuilt, is the most common reason iron deficiency returns within months of completing a course.
For severe deficiency, when oral supplements are not absorbed due to gut disease, or when haemoglobin is critically low and recovery needs to be faster, intravenous iron is given in a clinic or hospital. Your doctor makes this call based on your haemoglobin level, your symptoms and the underlying cause. Do not start iron supplements without medical advice, because the symptoms of iron deficiency overlap with several other conditions that need different treatment, and because excess iron accumulation is itself toxic.
Iron-rich foods in the regional diet
Dietary iron comes in two forms with very different absorption rates. Haem iron, found in red meat, poultry and fish, is absorbed at around 15 to 35 percent of the amount present, regardless of what else you eat with it. Non-haem iron, found in lentils, chickpeas, beans, dark leafy vegetables and sesame, is absorbed at only 2 to 10 percent under ordinary conditions, though pairing it with vitamin C-rich foods brings that figure up considerably.
The regional kitchen is genuinely well-stocked with iron sources. Liver and kidney are among the most concentrated animal sources available anywhere and are widely eaten in the region. Lentils, fava beans and chickpeas eaten daily provide meaningful amounts of plant iron. Parsley, tahini, dried apricots and sesame seeds contribute further. The problem is not a shortage of iron in the food but the widespread habit of drinking tea with a meal or within minutes of finishing it, a pattern that can cut iron absorption from plant foods by more than half.
If your diet relies mainly on plant iron sources, try adding lemon juice, fresh tomatoes or green pepper to your meals, and delay your tea by at least an hour after eating. These small and realistic adjustments to daily habits can meaningfully improve the iron your body actually absorbs from the food you are already preparing, without requiring any expensive supplements.
When to see a doctor about your ferritin result
Do not wait if your ferritin comes back below 12 ng/mL, or if it sits in the borderline range of 12 to 30 ng/mL and you are experiencing symptoms such as severe unexplained tiredness, marked hair loss, dizziness, or breathlessness with light effort. The combination of a borderline result and active symptoms deserves investigation rather than a wait-and-see approach.
An unexpectedly high ferritin above 300 ng/mL, in the absence of a recent infection or known inflammatory condition, also warrants a follow-up appointment to understand the cause rather than simply reassure yourself that your iron is plentiful.
Women planning a pregnancy, those who are already pregnant or breastfeeding, and anyone with a history of heavy menstrual periods or chronic gastrointestinal blood loss should have their ferritin checked as a matter of routine, before symptoms appear, rather than waiting until fatigue or pallor forces the issue.
