Why are blood thinners prescribed?

What is INR and how do you read your result?

INR stands for International Normalized Ratio. It is a standardised measure of how long it takes your blood to clot compared to a person not on any anticoagulant medication. A person not taking warfarin has an INR of approximately 1. The higher the INR, the less readily your blood clots, and the greater the risk of bleeding.

Foods that change how warfarin works

In the Arab kitchen, vitamin K is concentrated in fresh parsley and coriander used in large quantities, as in tabbouleh or certain stews, dark leafy vegetables, and some herbal preparations. Green tea also contains vitamin K. The practical approach is to eat what you normally eat and avoid large fluctuations. Medicines that interact with blood thinners

Dozens of commonly prescribed medicines raise or lower the effect of warfarin. Some increase its activity and raise bleeding risk; others reduce it and leave you less protected. Garlic, ginger, fish oil and ginkgo biloba all have antiplatelet effects that add to anticoagulant risk. Even a dose of aspirin bought without a prescription can meaningfully increase your bleeding risk if you are already on warfarin or a DOAC.

Warfarin versus DOACs: which is better for you?

A large multinational cohort study in Annals of Internal Medicine in 2022 comparing the four main DOACs found modest differences between them in specific outcomes, underscoring that choice should be individualised.

DOACs are not universally superior. They are contraindicated in people with mechanical heart valves, where warfarin remains the only evidence-based oral anticoagulant. Patients with severe kidney disease may not be able to take some DOACs safely. The absence of routine monitoring, while convenient, also means a missed dose or non-adherence goes undetected. And in settings where a reversal agent for a bleeding emergency may not be available, the lack of a universal antidote for some DOACs is a real consideration.

Bleeding warning signs

Bleeding is the main side effect of all anticoagulants. Most bleeding that occurs on these medicines is manageable, but some situations are emergencies. Knowing which is which helps you act appropriately rather than either panicking over a bruise or waiting too long with something serious.

For major surgery, the surgeon and anaesthetist decide together when to pause the anticoagulant before the operation and whether bridging anticoagulation with injectable heparin is needed during the pause. Living well on anticoagulants: travel, activity and daily life

Most people on long-term anticoagulation live a full and active life. The adjustments required are real but not limiting. Understanding them makes adherence easier and reduces anxiety about the medication.

When travelling, carry your medicine in hand luggage rather than checked bags. Carry a brief medical card in your wallet or phone stating the drug name, dose, and indication. If you travel to a country where your specific DOAC brand may not be available, take a sufficient supply for the entire trip plus a reserve. If your INR is due to be checked during a trip abroad and INR testing is not easily accessible, discuss this with your doctor before travel and agree on a plan.

Physical activity and exercise are encouraged, not contraindicated. Regular walking, swimming and moderate exercise pose no additional risk at therapeutic INR levels and have cardiovascular benefits. High-contact sports or activities with a significant fall risk need discussion with your doctor, particularly if INR is running at the higher end of your range. The answer is rarely to stop all activity: it is to choose activities that fit your current level of risk.
